Long Term Conditions
LTC is a service which offers support for adults diagnosed with long term health conditions and who are experiencing symptoms of depression, anxiety and/or stress. LTC is delivered by Low and High intensity CBT practitioners.

We deliver a range of interventions that are tailored to the needs of the client.
- CBT (Cognitive Behaviour Therapy)
- Health & Wellbeing programmes including self-guided support
- Wellbeing groups or coping groups
- Information and support in living with LTC and MUS
Our waiting times are 2 to 4 weeks.
Our clinically trained IAPT practitioners (Talking Therapists) and Health Psychologists, located within or very close to the GP’s surgeries deliver this service. All therapist are fully qualified with the LTC top training from Sheffield University.
The referrals will be generated by working with GP’s and through the MDT pathway. We are already part of the OMNIA MDT in the East. This has been an effective way of working to identify the needs of service users for the most appropriate therapy.
We accept self-referrals, this will be done via an assessment to ensure the service users are appropriate for the LTC pathway, confirming details with the GP before an intervention is proposed.

The conditions we support through LTC pathways are;
- Chronic obstructive pulmonary disease
- Cardiovascular disease (CHD)
- Musculoskeletal problems (Chronic pain).
- Chronic Kidney Disease
- Irritable bowel syndrome
- Chronic Fatigue Syndrome
- MUS not otherwise specified
Our service delivery model has the standard baseline of the Integrated IAPT treatment pathway with an active social prescribing strand.
